If you believe your info has changed please let us know. Maybe you disconnected your house phone? Maybe your son or daughter was your emergency contact but they’ve since moved. Whatever the case please take a few minutes to complete an updated form.

You may also pick up a blank printed copies on the table as you enter the atrium, where there is also a box where printed copies can be returned.

- Print it, fill it out and mail it to the church.
